HOW TO BEGIN

A Person-Centered Process

  1. Initial Contact

Contact our team to ask about services, current program availability, and possible next steps.

  2. Needs and Eligibility Review

The organization reviews available information to determine whether an available program may be appropriate.

  3. Individualized Plan

When services are appropriate, care is organized around the individual’s needs, strengths, and goals.

Program availability and clinical appropriateness are reviewed individually. These steps do not guarantee admission.
